Includes:
- consultation on surrogate maternity,
- initial acquaintance with the candidate for the surrogate mother, personal data check-up, offer of candidacy,
- candidate’s state, food, transportation care during the primary compulsory medical examination,
- organization of the primary compulsory medical examination and the results summing up,
- candidate’s psychological state’s assessment and consultation.
“STANDARD” PACKAGE OF SERVICES
Includes:
- consultation on surrogate maternity,
- initial acquaintance with the candidate for the surrogate mother, personal data check-up, offer of candidacy,
- candidate’s state, food, transportation care during the primary compulsory medical examination,
- organization of the primary compulsory medical examination and the results summing up,
- candidate’s psychological state’s assessment and consultation,
- control over state order assigned check-ups and procedures during the surrogate mother’s pregnancy period and organization of additional examinations carried out by leading specialists as per instructions,
- insurance of the surrogate mother’s life and health,
- legal consultation on the relationship between biological parents and surrogate mother candidate,
- legal registration of the necessary postnatal documents.
“RESPONSIBLE” PACKAGE OF SERVICES
Includes:
- SM (oocytes, sperm donor) selection and preliminary interview upon request,
- SM personal questionnaire filling and data checking:
- information on child(children),
- information from place of residence,
- information оn previous convictions…,
- organization of SM , sperm, oocytes donors’ compulsory check-ups,
- checking of information on the previous pregnancy(-ies) of SM,
- checking of personal information, private life and lifestyle of SM,
- guiding of biological parents or SM at the medical institutions, organization of visits,
- control over the SM lifestyle and other issues against the contract during the pregnancy period,
- SM support in private life (hiring of an assistant),
- apartment renting for SM,
- organization of additional services and check-ups not assigned by state order (upon instructions or request) during the pregnancy period,
- control over medical prescription implementation (during the treatment),
- provision of SM medical examination and consultation performed by the leading specialists of the medical institution,
- overall legal services provision of SM and customer’s relationship, documents registration concerning child’s birth,
- SM health insurance,
- insurance of the customer’s material expenditures,
- presentation of report on the performed tasks (once a month or once a quarter).
