- Originals and copies of marriage certificate
- The copy of sealed license (signed by the executive) for medical clinic where the artificial insemination is going to take place
- Notice from the medical clinic about extracorporeal fertilization procedure, fetus quantity also surrogate mother’s and biological parents’ personal information.
- Agreement stating surrogate mother’s and her spouse’s (if applicable) acceptance of extracorporeal fertilization procedure and surrogacy
- Agreement stating biological parents’ acceptance of extracorporeal fertilization and surrogacy
- Certificate of birth given by medical center (maternity)
- Agreement that biological parents are stated in birth certificate
- Petition for newborn’s residency submitted by biological parents
